Summary
The 2024 ILCOR CoSTR is the annual consensus synthesis across BLS, ALS, pediatric and neonatal life support, education, and first aid, using GRADE-based evidence review. ED-relevant recommendations include avoiding CPR delays merely to move a patient to a firmer surface, targeting an adult post-ROSC MAP of at least 60-65 mm Hg, treating post-arrest seizures without routine prophylaxis, and selected pediatric and neonatal updates.
Why it matters
For adult post-arrest ED care, the practical signal is to maintain MAP at least 60-65 mm Hg and treat seizures when present rather than giving routine prophylactic antiseizure medication. This is historical annual guidance; the 2025 ILCOR CoSTR contains newer evidence and recommendations.
Study at a glance
- Design
- Consensus statement
- Population
- Adults, children, neonates, and first-aid or resuscitation populations addressed across the six ILCOR task forces.
- Sample size
- Not applicable
- Intervention
- 2024 ILCOR consensus treatment recommendations derived from systematic and structured evidence reviews.
- Comparator
- Prior ILCOR recommendations or existing resuscitation practice; this is not a single comparative clinical trial.
- Primary endpoint
- Consensus treatment recommendations across cardiopulmonary resuscitation and emergency cardiovascular care.
Main findings
- CPR on a firm surface remains important, but moving a patient should not create avoidable delay in CPR initiation.
- After ROSC, an adult MAP of at least 60-65 mm Hg is suggested; pediatric post-arrest systolic pressure should be above the 10th percentile for age.
- Treatment of seizures after ROSC is suggested in adults and children, while routine seizure prophylaxis is not suggested.
- A good-practice statement supports ECMO only in very select pediatric pulmonary hypertensive crises, and delayed cord clamping for at least 60 seconds is recommended for preterm infants not needing immediate resuscitation.
Limitations
This is an umbrella consensus spanning many resuscitation domains, and evidence certainty and recommendation strength vary substantially by topic. The official society summary and PubMed abstract were reviewed rather than every underlying systematic review or full recommendation table.
Recommendation strength
ILCOR consensus on science with treatment recommendations using GRADE; recommendation strength and certainty vary by topic.
